What does it mean when we say these words?
Converted: to bring over from one belief, view or party to another or to go from a life of sin to a life of serving God.
When we go to the altar, we ask God to forgive us of our sins, when we believe that he has done that, we have been converted from our old life in sin, to begin a new life with God. We have been washed of all our sins and given a clean heart with sins gone.
I John 1:7: But if we walk in the light, as he is in the light, we have fellowship one with another, and the blood of Jesus Christ his Son cleanseth us from all sin.
I John 1:9: If we confess our sins, he is faithful and just to forgive us our sins, and to cleanse us from all unrighteousness.

Sanctified: set apart as or declare Holy, consecrated.
It is the renewal of our fallen nature by the Holy ghost, received through faith in Jesus Christ, whose blood of atonement cleanseth us from all sin, whereby we are not only delivered from the guilt of sin but we are washed form its power and are enable through grace.
After your sins are forgiven, you need to get the Holy Spirit, sanctified, in order to keep your mind on God, you have to completely give your life to God, let him rule in your life daily, how you talk and treat others.
